How You Access This Program
You need a referral from a county case manager to access our Supported Living Program:
- Contact your case manager/services coordinator in your county’s Developmental Disabilities Office.
- Tell them you want to pursue Supported Living services and would like to have a referral packet sent to Community Vision.
How This Program Works
Community Vision’s Supported Living Program supports people with a wide range of requirements and goals.
We assist each person with finding staff for their direct support team. It is important that direct support team members are a good match for the person they are supporting.
The team and the supported person work together to create a customized service plan. This plan meets the supported person’s needs and wishes.
Direct support staff can offer services from a few hours per week to many hours per day. They provide basic personal support services that include, but are not limited to:
- Support with meals, hygiene, and mobility
- Budgeting and bill paying
- Getting to and from daily errands
- Meeting short- and long-term goals
In addition to direct support, Supported Living includes support in the following areas:
- Coordinating daily schedules
- Financial and health oversight
- Building community connections
- Coordinating medical care
- Ensuring consistent communication with the person and their whole support team
The individual and their team collaborate on setting up systems for success in these areas.
Supported Living is funded through Oregon Developmental Disabilities Services.

Tony is a lifelong Star Trek fan. Community Vision has supported him for many years. When our Supported Living team saw that the Oregon Symphony was hosting an event called “Star Wars vs. Star Trek,” Tony immediately came to mind.

People currently supported by Community Vision should contact your DSP (Direct Service Professional) or your Supported Living Coordinator.
