Family Support

The Family Partnership Program provides support to individuals with disabilities who live in their family’s home, giving their family members the opportunity to take time for personal, professional, or family matters. The program provides quality supports at home, as well as in the broader community – stewarding access to community resources to assist with support services, employment opportunities, housing and self-determination.

Community Vision staff will assist the individual and their family to transition into one-on-one Supported Living and/or the move from high school to employment services as their loved one’s needs change, while relieving the pressure on families by providing oversight of staff working in the home. The Family Partnership Program aims to assist the individual and their family to provide guidance to best utilize their state funding so that their loved one has a stable system of support that will last a lifetime.
