Youth and Family

Community Vision provides youth approaching transition ages and families providing support to individuals experiencing disabilities with access to highly specified programs that can improve their experience. We focus on providing access to resources and guidance that create paths to more sustainable, fulfilling lives.

Dream Builders

Dream Builders Alliance is a transition-age program that provides young adults experiencing disability with mentorship, and equips their families with resources to support them in building toward their future. Young adults are matched with a one-on-one mentor to help them set goals and develop skills. Parents, guardians, and other family members engage in a series of education workshops to help them prepare for their youth’s transition to adulthood.

Family Support Program

The Family Partnership program provides support to individuals experiencing disabilities who live in their family’s home. Community Vision supports these families by stepping in and providing quality support services in and out of the home, giving their family members the opportunity to take time for personal and professional matters.
