Supported Living Facilitator

Our Facilitators manage one individual’s household. The Facilitator schedules staff, doctor’s appointments, and activities at that house, provides daily support at home and in the community, as well as overseeing the paperwork done by other staff members. The Facilitator should be an experienced caregiver. Supervisory experience is helpful.

Qualifications

Ability to work effectively for and with individuals who receive our support.

Ability to train, evaluate and supervise household staff.

Ability to arrange staffing for the individual supported within the budgeted hours assigned for the home, including being flexible in your own work hours to ensure staffing coverage.

Ability to learn and understand the systems the individual supported uses for services i.e., Disability Services Office, Oregon Health Plan, Housing Authority, etc.

Own a car with liability insurance and have a valid State of Oregon driver’s license.

At least 18 years of age.

Duties

  • Assist people who receive our services to grow in their relationships with others in their community.
  • Supervise day-to-day activities as needed (dressing, grooming, bathing, eating).
  • Supervise household maintenance (cleaning, shopping, meal planning and preparation).
  • Supervise any mobility issues as needed (wheelchair maintenance, mobility in the home).
  • Supervise health maintenance (administering of medications and treatments, provide basic first aid, schedule and attend medical appointments).
  • Supervise safety and security (Check emergency equipment weekly, check for potential safety hazards on a daily basis, assist with contacting appropriate person(s) for needed emergency repairs).
  • Managerial Duties (Schedule support staff as needed on a daily basis, maintain timesheets and a master calendar of billable hours, maintain an ongoing record of employee performance for those you supervise, develop personnel evaluations in conjunction with the Coordinator/Assistant Director, complete a monthly House Review List concerning all paperwork and safety issues within the home.
