Community Vision provides Supported Living services to individuals who wish to live in their own home with one-on-one support.
We support each person with developmental disabilities in the way that fits their life; providing supports that are unique to their needs, goals and dreams. This helps the individual live, work and thrive in the communities of their choice.
In Oregon, there are various models of support available to people with developmental disabilities. Group living settings are commonplace but cannot offer true choice and independence to each individual. Community Vision assists individuals in selecting the options that best fit their needs and support their independence goals.
How it works
Community Vision supports individuals with a wide range of support needs – from a few hours per week to many hours per day. The individual and the support team work together to create a customized vision of support that suits their needs and wishes. We then assist the individual to find support staff that work well in their life and provide basic personal support services that include, but are not limited to:
- Support with meals, hygiene, and mobility
- Budgeting and bill paying
- Getting to and from daily errands
- Coordinating medical care
- Meeting personal goals and reaching for their dreams
Eligibility
We accept requests for new participants on an ongoing basis.
To learn more about obtaining services through the organization, contact your case manager/services coordinator in your county’s developmental disabilities office. Inform them that you would like to pursue Supported Living services and would like to have a referral packet sent to Community Vision.
