New Participants
We accept requests for new participants on an ongoing basis, but all requests must be made through a County Disabilities Office.

If you already have been approved for support and are interested in Community Vision, please let your County Case Manager (Services Coordinator) know that you would like staff support through our Supported Living Program. Your Case Manager can then contact Community Vision to explore support options.

If you have not yet qualified for services, the County Disabilities Office will be able to help with the eligibility requirements and the intake process.
