We serve individuals who qualify under DD 54.  Make an appointment with your County Disabilities Office case worker, and request that they send a referral packet to Community Vision, Inc.  If you do not have a case worker or you don’t know if you are eligible for DD 54, contact your county’s DD Program and ask for intake/eligibility to begin the process.  In Multnomah County the number is 503-988-3658.
